SUMMER TWIRL FEST - Registration

REGISTRATION AND FEES ARE NON-REFUNDABLE AFTER SESSION BEGINS

I/We, the parents of the above named child, hereby give our consent for participation in the above capacity and do claim that she/he is in perfect physical condition to participate in said activity. Furthermore, I/we hereby give my/our consent to her/his participation in Twirl Fest 2026*

I/We assume all risks and hazards incidental to such participation including transportation to and fromthe activities; and I/we do hereby waive, release, absolve, indemnity and agree to hold harmless the Sparklers/Imperial Knights, Inc., *

The USTA, associated organization, the organizers, sponsors, supervisors, participants and person transporting my/our

daughter/son to or from activities, for any claim raising out of an injury to my/our daughter/son, except to the extent and in the

amount covered by accident or liability insurance.*

Family and friends are not permitted in gym during session times.*

Parents are expected to pick up children promptly at the end of class; coaches cannot wait at the building after class.*

No smoking and drinking permitted on building grounds*

I/we have received and read the rules of Twirl Fest 2026 and will adhere to them to the best of my/our abilities.
